| CNA and CMT (Personal Care, Companionship, Transportation Coordination, Care Coordination, Meal Preparation) | Certified Nursing Assistant provides delegated nursing tasks, companionship, personal hygiene assistance, and minor housekeeping (up to 20% of assignment hours). The Certified Medication Technician administers medication. | $35.00 Hr. (Four Hour Minimum) |

Medicaid bills all Services to the Client/Patient and reimbursement and Private Pay Insurance Companies are the responsibilities of the Client/Patient. Services are billed on a weekly basis.
