Initial Assessment
Director of Nursing/RN Supervisor will conduct an initial assessment and implement a Plan of Care (POC).

One-time fee
RN or LPN Skill Nursing Service (Skilled Nursing Services)
Personal care and skilled service by a licensed nurse.
$70.50 Hr.
(Four Hour Minimum)
CNA and CMT (Personal Care, Companionship, Transportation Coordination, Care Coordination, Meal Preparation)
Certified Nursing Assistant provides delegated nursing tasks, companionship, personal hygiene assistance, and minor housekeeping (up to 20% of assignment hours). The Certified Medication Technician administers medication.
$35.00 Hr.
(Four Hour Minimum)
HHA (Homemaking, Respite Care)
Bathing, personal care, and companionship. May include housekeeping, meal preparation, or errands.
$40.00 Hr.
(Two Hour Minimum)
Mini bath
Bathing and personal care by an aide.
$50.00 Hr.
(Two Hour Minimum)
Multiple Shifts Required
Skilled Nursing and Aide Service when a 24-hour period is required.
$400.00 Day
(Per 24-hour period)
Medicaid bills all Services to the Client/Patient and reimbursement and Private Pay Insurance Companies are the responsibilities of the Client/Patient. Services are billed on a weekly basis.
